Voorhees was shot down 8 May 1944 in B-17 #42-38190 and Killed in Action (KIA) on a mission to Berlin. His aircraft was attacked by enemy fighters and exploded in mid-air, 9 members of the crew were unable to bail out and were killed in the incident. Their bodies were recovered on the ground by German authorities.
